The Mercer County Department of Human Services, Office on Addiction Services, is accepting proposals for the period of one-year (January 1, 2023 – December 31, 2023) with the option to extend for an additional year (January 1, 2024 – December 31, 2024), to provide Social Security Income SSI/Social Security Disability Income SSDA Outreach Access and Recover (SOAR) Case Management services for indigent Mercer County residents in the amount of $200,000.00 annually (State Funds). Up to two awards may be made to eligible applicants.

Funding will be made available in the service category:

  • Addiction Case Management Services within the Homelessness System
    • SOAR Case Management

Eligible applicants must have licensure from the New Jersey Division of Mental Health and Addiction Services (DMHAS). Up to two awards may be made to eligible applicants.
